For all new patients, it is recommended that one complete all the necessary new patient forms prior to coming in to the office. You now have 2 options available.
| Option 1: Click on the link below to print out forms to complete at home. You may fax the forms to 954-421-3201 along with copies
of both sides of your insurance card(s).
In addition, bring in the completed forms to our office along with your insurance card(s) and a picture ID. If you do not fax this information at least 2 business days in advance, we will have to enter this information in manually before you are seen. This entry may take 20 minutes or more as your insurance information must be validated prior to your being seen. |
Option 2 (recommended): Click on the link below to complete your registration forms online. This information will be forwarded to our office for immediate entry. This will save you time when you arrive to our office. You will have to sign some documents when you arrive, but this should not take much time. These electronic forms are sent by e-mail and occasionally we do not receive them. Therefore, please print out your completed forms and bring them with you to the office along with your insurance card(s) and a picture ID. |
