It is recommended that all new patients read this information completely. We have two office locations in Broward County, Florida.
Call our office at 954-755-1612 to make an appointment for either office location during normal working hours. Our hours do change from time-to-time and the table below should serve as a reference. It is recommended that all patients call for an appointment. Walk in appointments are not allowed.
Cancellation Policy for Appointments We require 24 hours notice or 1 business day's notice, whichever is greater, to cancel an appointment. Failure to provide this notice will result in a service charge of $55 or more for an appointment and $125 or more for a procedure, which will not be covered by insurance. We understand that rare instances may occur when it would not be possible to give such notice. We ask that one be respectful of our time and of other patients who are trying to get an appointment by adhering to this cancellation policy.
Insurance Plans accepted We accept the following plans:
We do NOT accept any Medicare HMO plan other than Humana HMO and Careplus (must have Dr. Ginnis or Dr. Arena as a primary physician) We do NOT accept Aetna™ HMO, BC/BS™ HMO, Health Options™ HMO, Vista™, or AvMed™. Although we are not contracted with Aetna Medicare plans, we are able to see patients with this insurance and bill your insurance directly. We are able to see many patients on Medicare PPO plans and submit to your insurance. Please call if you have any questions. It is your responsibility to know your insurance benefits. If you have any questions regarding your coverage, we recommend that you contact your insurance company before your visit. Most insurance cards have a customer service phone number on the back of one's insurance card. COMPLETED FORMS: It is recommended that one fill out all the medical history forms and registration forms in advance--click on "Forms" tab on left side of this page. INSURANCE CARD: Bring your insurance card with you, even if we do not accept your insurance as we will try to send any specimens to a laboratory that does accept your insurance whenever possible. We will be unable to do this if you do not bring your card or a copy of both sides of your card. MEDICATION LIST: Bring a complete list of your medications and allergies. If you do not know them, please contact your physician or pharmacy to get a complete list.
We will always do our best to inform you if a procedure is not be covered by your insurance before the procedure is performed. There is ALWAYS an additional charge for ANY and ALL procedures that are done in addition to the visit charges. We will assist in submitting to Medicare secondary insurance companies, although all Medicare patients will be responsible for their annual deductible (currently $124) and 20% co-pays as mandated by Medicare guidelines. We are not responsible for submitting to Medicare secondary insurance and we may do so as a courtesy. Some persons have a primary insurance that they are using as a Medicare secondary. This is very different from a Medicare secondary and a provider often must be contracted with that company in order for the participant (patient) to receive benefits. It is important that you know and understand your benefits. All payments are due in full at the time of service.
Full Body Examinations/Skin Cancer Screenings Skin cancer screenings are recommended for all patients. Full body examinations must be scheduled in advance. It is recommended that one does not wear make-up to the screening.
We offer a topical numbing cream (LMX-4 or ELAMax 4) that must be applied 1 hour before the procedure. You must have this in advance--before your scheduled procedure. This makes the procedure a little more comfortable. If you would like to use the numbing cream, you may purchase it at any pharmacy without a prescription or at our office. We cannot wait for patients who did not apply the medication before their appointment. Patients should not take aspirin for at least 10 days before their procedure. Patients who are taking aspirin or other blood thinners (i.e. Plavix®) under the recommendation of a physician should contact that physician for approval to discontinue the blood thinner. Patients should not take anti-inflammatory medications (i.e. ibuprofen, Aleve®, Advil®, Motrin®), Vitamin E or St. John's wort for at least 3 days before the procedure. If any of these products are taken, your procedure may be cancelled and a service charge may apply. Please contact the office if you have any questions.
Patients often wish to have cysts removed due to cosmetic reasons or because they are bothersome. All cysts must be evaluated first. Cysts will not be removed until they are examined at a regular visit. Removal of the cyst will be performed on a different day than the evaluation. Tender, swollen, hot cysts can be drained at the time of the initial visit, but they will often recur and may require surgical excision at a later date.
Most minor growths (tags and most raised growths and moles) can be removed at the time of the initial visit. More extensive removal, such as those requiring stitches, will need to be evaluated first, and then scheduled for a later date. Those with a few skin tags can usually be removed at any visit. Those with many skin tags may need to have them scheduled for removal at a later date after the first visit. Some health plans (i.e. Humana HMO) require a referral to remove skin tags BEFORE the visit.
