Our office requires one full business day’s notice to cancel an appointment. This is required as a courtesy so that other patients whom are waiting to be seen can be scheduled in your time slot. Failure to provide one full business day’s notice will result in a cancellation charge of:
$25 for office visits and consultations
$125 for routine surgical procedures (excisions and minor procedures)
$250 for Mohs’ Surgery
For major procedures requiring a deposit before the procedure, information about the cancellation policy specific to those procedures will be given in writing.
Florida Law requires the consent of a parent (mother or father) or a legal guardian for the treatment of a minor.
For NEW minor patients, our office requires that a parent or legal guardian be present for the first visit –– no exceptions.
For follow-up visits, a parent or legal guardian may authorize/consent to the minor coming for follow-up visits. During these visits no prescriptions will be given, except refills, which must be done electronically to your pharmacy. Additionally, we will not be able to perform any procedures without a parent or legal guardian present. If a NEW prescription or procedure is recommended, the MINOR patient must return to the office with the parent or legal guardian.
